Advanced Qualification In Headache Medicine Certificate (AQH©) New Application

Welcome to the AQH© Examination Application (for Fall 2025 cohort only).

Eligible candidates are licensed healthcare providers with prescriptive authority, independently evaluating and managing headache patients.

Part A: Before completing your online application, please review all AQH© certification fees, terms, and policies at: https://headaches.org/aqh

In the signature portion of the application process, you will be asked to attest that you understand and agree with all policies.

Part B: To complete your AQH© application, please be prepared as the following requirements will need to be uploaded: (1) a copy of your current medical license, (2) curriculum vitae (CV), (3) colleague recommendation letter, and (4) documentation of at least 50 CME units related to headache medicine from the past three years, with ideally 20 of those CME in the last year.

Part C: Be prepared with your choice of sitting for the examination either in a test center or via online proctoring.

Part D: After completing, uploading requirements, signing and submitting the form, you will be automatically directed to a secure site to pay the $875.00 application fee.

Part E: All applications will be reviewed by the AQH© Review Committee on a rolling basis.

Part F: Once your application is reviewed, you will be notified of the results of the review.

If your application is approved, you will be contacted regarding your candidacy with (1) a link to the pre-examination review guide, and (2) instructions and timing to register for the date and time to sit for the examination.

The AQH© Examination is a proctored online computer-based, 150 questions in 3 hours and 20 minutes (200 minutes including optional break). You will have the option of utilizing a test center, or using an online proctor with your computer at a location of your choice.

Once you start the examination, you must complete it. You cannot start, save, and re-enter, so plan your availability for the full 3 hours and 20 minutes, plus an additional 30 minutes for required ID verification and any travel time if applicable.

If you select the online proctoring option, it will be your responsibility to ensure stable Wi-Fi, and computer readiness ahead of time to ensure your computer meets minimum requirements for access.

Full Review: $375.00 non-refundable when applicant moved forward to full review and is determined eligible to sit for examination.

Non-Disclosure Agreement Policy

At the beginning of the examination, you will be required to affirm that you have read and accept the terms of the NHF Non-Disclosure Agreement (NDA).

NOTE: You are required to accept the NDA within the first 5 minutes to start the examination. If you do not accept, you will not be allowed to take the exam, and you will forfeit the $500 examination fee.

I understand that all documents and examination questions are, and shall remain, property of the National Headache Foundation. I also understand and agree that I can be disqualified from taking or completing the AQH© Examination, or from receiving examination scores, if the National Headache Foundation determines either through proctor observation or statistical analysis I have engaged in fraudulent, collaborative, disruptive, or other inappropriate behavior during the administration of this examination.
